Steps to report a lost card
Note:  Visit the Lost Card information page for more details regarding lost cards.  It is critical to report a lost card immediately as this will lock your account and prevent unauthorized use of your plan balances.

• Click the REPORT LOST CARD link
• Click Yes to deactivate your card
• What to do next
• You will need to visit a campus ID office to request a replacement (a $10 fee will need to be paid at a campus Business Office)
